LIBERTY PROPERTY DAMAGE SERVICE AGREEMENT

 

I/We understand that Liberty Property Damage, LLC ("LPD") charges a 1/3 contingency fee for its property damage assistance services.

By submitting the intake form, I/we authorize LPD to provide property damage assistance in connection with my/our claim and agree to pay the service fee.

Payment Options
The service fee may be paid by any of the following methods:

1. Deduction from my/our property damage settlement proceeds;


2. Direct payment to LPD by debit card or credit card.
   
   Acknowledge:
   I/We understand and acknowledge that LPD does not guarantee any specific result, recovery, settlement amount, or time frame. LPD will use its best efforts in providing property damage assistance.
   
   Authorization:You further authorize LPD to generate an Authorization Form to represent my/our interests in investigating and negotiating with the insurance company or at-fault party.

This Agreement shall be governed by the laws of the State of Arizona, without regard to its conflict of laws principles. Any disputes arising out of this Agreement shall be brought in the state court located in Maricopa County, Arizona.

This Agreement constitutes the entire agreement between the parties. No modification of this Agreement shall be effective unless it is in writing.

By submitting the intake form, I/we acknowledge that I/we have read, understand, and agree to the terms of this Agreement.
