I/We understand that Liberty Property Damage, LLC., ("LPD") charges a $120 fee for its property damage assistance services.
By submitting the intake form, I/we authorize LPD to provide property damage assistance in connection with my/our claim and agree to pay the $120 service fee.
Payment Options The $120 service fee may be paid by any of the following methods:
Deduction from my/our property damage settlement proceeds;
Direct payment to LPD by debit card or credit card.
Payment through a lien or assignment against my/our personal injury claim.
I/We understand and acknowledge that LPD does not guarantee any specific result, recovery, settlement amount, or time frame. LPD agrees only to use its best efforts in providing property damage assistance.
This Agreement shall be governed by the laws of the State of Arizona, without regard to its conflict of laws principles. Any disputes arising out of this Agreement shall be brought in the state court located in Maricopa County, Arizona.
This Agreement constitutes the entire agreement between the parties. No modification of this Agreement shall be effective unless it is in writing.
By submitting the intake form, I/we acknowledge that I/we have read, understand, and agree to the terms of this Agreement.
